Behavioral Health Case Manager (Southwest)

Job Summary We are seeking a dedicated and compassionate Case Manager to join our Behavioral Health team at Merrionette Park. The Case Manager will provide comprehensive services tailored to a diverse client population, assist with emergency and crisis situations, and work across office, client homes, and community locations, ensuring culturally competent service delivery for individuals from diverse backgrounds. Key Responsibilities Conduct thorough client assessments and evaluate problem and service needs. Develop written client service plans specifying goals and services. Supplement service plans to meet client needs. Provide supportive help, advocacy, and case management to individuals and families coping with environmental and interpersonal issues. Collaborate with community resources and help clients utilize them. Work within contract, grant, and program requirements including meeting timelines and reporting requirements. Engage clients in challenging settings for learning and developing healthy behaviors and coping skills. Reassess and monitor client situation and adequacy of service plan. Collaborate and participate with other program staff in planning and implementation of service plans. Handle emergency and crisis situations within target population following consultation with supervisor. With supervisory approval, authorise limited financial assistance to clients. Work with community groups as needed. Work effectively with diverse staff and service population. Other duties as assigned. Salary $42,000 to $47,000 annually, depending on skill and experience. Qualifications Bachelor’s Degree in Psychology, Social Work, or a related field. One year of case management experience in a social service or health care setting. Certified Alcohol and Drug Counselor certification (preferred). Ability to work evenings as needed. Additional Requirements Pass all relevant background checks and screenings. Travel / Transportation Driving for work required with transportation of clients preferred in a personally owned vehicle. Physical Demands Require sitting, standing; frequent talking, hearing, hand use; occasional bending, light lifting up to 30 lbs.
