Job Details

• Provide comprehensive primary care in an outpatient setting
• Deliver care to a mission-driven, high-need patient population
• Collaborate with behavioral health, substance use, and case management teams
• Contribute to quality initiatives and improved patient outcomes
• Help refine clinical workflows and care delivery models as the clinic expands

Requirements

• MD or DO, licensed or eligible for licensure in New York
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Experience in community health, FQHC, or underserved populations preferred
• Passion for serving vulnerable populations and contributing to a growing care model
