Our model focuses on addressing both medical and social needs through a collaborative, mission-driven approach. Job Details
- Provide comprehensive primary care in an outpatient setting
- Deliver care to a mission-driven, high-need patient population
- Collaborate with behavioral health, substance use, and case management teams
- Contribute to quality initiatives and improved patient outcomes
- Help refine clinical workflows and care delivery models as the clinic expands
Requirements
- MD or DO, licensed or eligible for licensure in New York
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Experience in community health, FQHC, or underserved populations preferred
- Passion for serving vulnerable populations and contributing to a growing care model
