This role focuses on assessing patient conditions, administering treatments, and collaborating with healthcare teams to ensure optimal patient outcomes. Flexibility and the ability to adapt to an on-call schedule are essential, along with proficiency in participant assessment documentation specific to home care.

Responsibilities

• Assess patient health status and document findings accurately using Participant Assessment for home care
• Administer medications and treatments as prescribed
• Collaborate with physicians and healthcare team members to plan and implement patient care
• Monitor patient progress and report changes in condition
• Educate patients and families on health management and post-treatment care
• Maintain compliance with healthcare regulations and safety standards
• Respond promptly to patient needs during on-call shift assignments

Preferred Qualifications

• 1+ years of nursing experience
• Patient assessment expertise
• Care planning skills
• Patient education experience
• Clinical documentation proficiency
