Community Health Advocate & Outreach Specialist

Hackensack Meridian Health is seeking a Community Health Worker to join the John Theurer Cancer Center's outreach and clinical teams. The CHW will assess community health needs, assist with planning and materials, provide social support, patient advocacy, and coordinate health education for the JTCC catchment area.
