Job Title: Licensed Practical Nurse (LPN) – Long Term Care (SNF/LTC)

Location: Chambersburg, PA
Pay Rate: $1,350 – $1,650/week (based on experience & eligibility)
Shift: Night Shift (6:00 PM – 6:00 AM)
Hours: 36 Hours/Week
Duration: 13 Weeks (Extension possible)

"Navitas Healthcare, LLC" is seeking a compassionate and skilled Licensed Practical Nurse (LPN) for a Long-Term Care / Skilled Nursing Facility setting. The LPN will provide direct resident care across LTC, SNF, dementia, and rehabilitation units while ensuring safe, high-quality, and patient-centered care.

This role involves collaboration with RNs, physicians, CNAs, and interdisciplinary teams while maintaining compliance with clinical standards and facility policies.

• Provide direct nursing care in LTC, SNF, dementia, and rehab units
• Administer medications, treatments, and nursing interventions
• Monitor resident conditions and report any changes promptly
• Assist in care planning and implementation of individualized plans
• Provide dementia and Alzheimer’s care using proper communication techniques
• Support rehabilitation patients recovering from surgeries, strokes, or illness
• Perform wound care, IV therapy, and geriatric assessments as needed
• Maintain accurate documentation in EMR system
• Collaborate with interdisciplinary healthcare teams
• Ensure resident safety, dignity, and quality of life
• Float between units as needed
• Educate residents and families regarding care plans

• Active Pennsylvania LPN License or Compact LPN License
• BLS Certification (Required)
• Graduate of an approved Practical Nursing Program
• Minimum 1 year LPN experience in LTC/SNF setting
