About the Role

The Physical Therapist Assistant (PTA) provides physical therapy services to residents under the direction and supervision of a Physical Therapist. The PTA assists with implementing individualized treatment plans, monitoring resident progress, educating residents and families, and collaborating with the interdisciplinary team to support successful rehabilitation outcomes.

This position is ideal for a compassionate and motivated therapy professional who enjoys working with residents and wants to make a meaningful difference in their quality of life.

Essential Responsibilities
  • Provide physical therapy services under the direction of the Physical Therapist and in accordance with established standards of practice, company procedures, and Therapy Department guidelines.
  • Implement individualized treatment plans developed by the Physical Therapist.
  • Assist the Physical Therapist with resident evaluations and development of treatment plans as directed.
  • Conduct therapeutic interventions and activities according to the established plan of care.
  • Monitor residents' responses to treatment and communicate progress or concerns to the Physical Therapist.
  • Assist with adapting treatment techniques and activities based on resident response and established goals.
  • Establish and coordinate individual resident therapy schedules in collaboration with the Physical Therapist and interdisciplinary team.
  • Participate in resident, family, and staff education regarding physical therapy procedures, exercises, strategies, and recommendations.
  • Participate in discharge planning and help prepare residents for transition from therapy services.
  • Collaborate with nursing, therapy, medical staff, residents, and families to coordinate the resident's overall rehabilitation program.
  • Supervise Rehabilitation Aides, Physical Therapy Aides, students, and volunteers as directed and within applicable state practice guidelines.
  • Participate in Quality Assurance programs and initiatives as required.
  • Maintain accurate and timely documentation and billing in accordance with company and Therapy Department procedures.
  • Maintain knowledge of applicable laws and regulations governing rehabilitative services.
  • Maintain a safe environment and follow infection control, universal precautions, OSHA, and other applicable safety standards.
  • Report hazardous conditions, incidents, injuries, equipment concerns, and changes in resident condition to the appropriate supervisor.
  • Attend required meetings, in-services, training, and continuing education programs.
  • Maintain professional standards of appearance, hygiene, communication, and conduct.
  • Communicate respectfully and professionally with residents, families, coworkers, and other members of the healthcare team.
  • Adapt positively to changes in responsibilities, schedules, and assignments.
  • Maintain predictable attendance and arrive on time for scheduled shifts.
  • Perform other duties as assigned.
Qualifications
  • Associate's degree from an accredited Physical Therapist Assistant program.
  • Current Physical Therapist Assistant license in the state(s) in which services are provided.
  • Maintain all required certifications and licensure.
  • Commitment to ongoing continuing education and professional development.
  • Previous experience in skilled nursing, long-term care, rehabilitation, or geriatric care preferred.
  • Ability to work effectively as part of an interdisciplinary team.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to prioritize responsibilities and manage multiple resident needs.
  • Knowledge of applicable state and federal regulations related to physical therapy services.
  • Experience working with older adults and individuals with specialized rehabilitation needs preferred.
Physical Requirements
  • Ability to walk throughout the facility and therapy department throughout the workday.
  • Ability to stand for extended and repetitive periods.
  • Ability to sit and work at a computer for extended periods.
  • Ability to bend, stoop, squat, kneel, reach, and navigate stairs as required.
  • Ability to assist residents with transfers, mobility, therapeutic activities, and positioning.
  • Ability to lift up to 50 pounds .
  • Ability to safely operate and utilize therapy equipment and other required tools.
  • Ability to maintain the physical and visual abilities necessary to provide safe resident care and complete required documentation.
  • Travel may be required as needed.
  • Maintain predictable attendance.
