Primary Care Provider Role

Mon - Fri, primary care provider role managing chronic conditions of own panel. No weekends/evenings.

$7,500 sign on bonus + annual quality-based bonus program

Responsibilities:

The physician delivers primary health care and patient care management in a residential setting.

Qualifications:

• Experience in geriatrics or complex patients preferred
• Active/unrestricted medical license and DEA
• Board certified or board eligible
• EMR experience
• Must maintain a valid driver's license and good driving record

Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
