Physical Therapist (PT) – FT / PT
Green Acres Nursing & Rehab – La Plata, MD
$52–$54/hour (based on experience)
$$$ 2,500 Sign-On Bonus $$$$

Responsibilities:

• Evaluate and treat residents requiring physical therapy

• Develop and implement individualized treatment plans

• Set functional, realistic goals for patients

• Complete documentation (evaluations, progress notes, discharges)

• Collaborate with nursing and interdisciplinary team

• Educate patients and families

Qualifications:

• Graduate of an accredited Physical Therapy program

• Active Maryland PT license

• SNF/long-term care experience preferred (not required)
