CVS Health is hiring a Case Manager RN for a 100% remote role. You will assess, plan, facilitate and advocate for members to meet benefit plans and health needs. Work in collaboration with Medical Directors, manage discharge planning, and coordinate care across settings from your home state. The role requires an active RN license, 3+ years of acute care RN experience, and the ability to work 8:00am–4:30pm in the resident time zone with occasional late shifts.
