PRN Registered Nurse

Position Overview PRN Home Health Registered Nurse (RN) – Monday‑Friday. Serving Dorchester, Charleston, and Berkeley Counties. PRN Monday thru Friday (no weekend shifts). Key Responsibilities Participate in ongoing interdisciplinary assessment of the patient. Complete initial and ongoing comprehensive assessments of the patient’s needs. Participate in the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s). Help the patient clarify mutually agreed upon goals and incorporate them into the plan of care. Provide services ordered by the physician as indicated in the plan of care. Counsel and educate the patient and family on meeting healthcare–related needs. Initiate appropriate preventative and rehabilitative nursing procedures. Refer to other services as needed. Monitor and evaluate outcomes of patient care. Communicate with appropriate staff regarding changes in the patient’s needs. Supervise paraprofessional staff. Effectively communicate with other members of the interdisciplinary healthcare team to promote coordination of patient care. Communicate with all physicians involved in the plan of care and other health care practitioners. Participate in the Quality Assessment and Performance Improvement (QAPI) program. Participate in in‑services or training, as requested. Access personal health information (“PHI”) necessary to fulfill the above duties, subject to each organization’s definitions and policies. Benefits Competitive wages: $34.00–$38.00/hour DOE Weekly pay Mileage reimbursement 1:1 patient ratios Requirements Graduate of an accredited school of nursing and current RN license in SC or a compact license. Two years’ experience as a nurse, with one year of experience in home health preferred. Proficient computer skills and workplace software knowledge. Strong communication skills, compassion, and clinical judgment; ability to work well under stress and be creative. Ability to handle multiple changes based on patient needs and scheduling. CPR certification. Valid driver’s license and proof of insurance.
