365 Healthcare is seeking a Board Certified or Board Eligible Family Medicine Physician to join a busy, well-established outpatient primary care practice in Clinton, Iowa . This permanent opportunity offers a strong referral base, excellent clinical support, and outstanding work–life balance while allowing the physician to build a robust and sustainable patient panel.

Outpatient family medicine practice

Team-based model with physicians and advanced practice providers

Monday–Friday clinic-based care

Shared call rotation 1:4

Opportunity to build and customize a long-term patient panel

Established, loyal patient population with room for growth

Onsite laboratory services available

Strong specialty support for coordinated care

Physician-led practice environment

Provide full-spectrum outpatient primary care

Manage acute and chronic medical conditions

Develop and maintain a long-term patient panel

Participate in shared call coverage

Collaborate with APPs and specialty providers

Deliver patient-centered, community-focused care

Board Certified or Board Eligible in Family Medicine

Active Iowa medical license or willingness to obtain

Clean malpractice and licensing history preferred

Eligible for J-1 or H-1B visa sponsorship

Ability to commit to full-time outpatient practice

Recent NPDB self-query required at time of presentation

Two-year income guarantee: $290,100 annually

Sign-on and bonus package totaling $150,000 , plus productivity incentives

Moving allowance up to $15,000
