Responsibilities:

• Promote Elara Carings philosophy and mission to ensure quality of care.
• Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
• Design and implement individualized care plans based on thorough assessments.
• Conduct therapy practice within defined standards of care.
• Direct assistance of support staff to achieve patients functional goals.
• Document evaluations, treatment goals, and regularly update patient records.
• Provide direction and leadership to staff and promote physical therapy in the community.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Dependable vehicle, valid drivers license, and current auto insurance.
• Willing to travel 50% within branch/office coverage area.
