Case Manager - Travel

Duties:

• Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process
• Performs utilization management and quality screenings

Skills:

• In- Patient Case Management is required
• Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities
• Minimum 2 years of experience with acute inpatient hospital discharge planning
• Experience with subacute patients required
• 2 years of acute care experience strongly preferred
• Case Management, Managed Care.

Education:

• BSN required
• RN license required
• CCMC preferred

Shift:

• 8:00 AM 4:00 PM
• Rotating weekends: Every 4th or 5th weekend
• Holidays: One summer and one winter holiday
• 37.50 hours/weekly
• 5 days/week
