This role involves working in a home health setting, providing comprehensive care to patients needing rehabilitation, strengthening, and cardiac rehab. Key Responsibilities of the Physical Therapist

Provide home health services to patients with various diagnoses including orthopedic and generalized weakness
Utilize the Epic documentation system for patient charting
Perform patient assessments and create personalized rehabilitation plans
Collaborate with PTAs and other team members to ensure optimal patient outcomes

Requirements of the Physical Therapist

BLS Certification
Physical Therapist license - Must meet WV state requirements for practice
Open and positive attitude towards learning the Epic system
