Job Summary
The MDS Coordinator position at Vantage at West Springfield is responsible for coordinating and completing accurate resident assessments in compliance with state and federal regulations. The MDS Coordinator works collaboratively with the interdisciplinary team to support quality care outcomes and reimbursement accuracy.

MDS Coordinator Duties & Responsibilities

• Coordinate and complete MDS assessments and care plans

• Ensure accuracy, timeliness, and regulatory compliance

• Collaborate with nursing, therapy, and interdisciplinary teams

• Monitor documentation to support reimbursement and quality metrics

• Participate in audits and quality improvement initiatives

MDS Coordinator Compensation & Schedule

• Employment Type: Full-Time / Part-Time / Per Diem

• Pay Range: $65,000.00 - $90,000.00

MDS Coordinator Qualifications & Skills

• Active Massachusetts RN or LPN license

• MDS experience preferred

• Strong organizational and analytical skills

• Knowledge of long-term care regulations
