Key Responsibilities

• Evaluate patients in a home health setting to assess functional abilities and develop individualized treatment plans
• Provide occupational therapy interventions aimed at improving patients' ability to perform daily living activities
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to ensure coordinated and comprehensive patient care
• Educate patients and caregivers on therapeutic techniques and home safety strategies

Required Qualifications

• Current, valid Occupational Therapist licensure in the state of Iowa
• Graduate of an accredited Occupational Therapy program
• NBCOT certification (or eligibility)
• Experience providing occupational therapy services in a home health or similar setting
• Valid driver's license and reliable transportation for home visits

Preferred Experience / Skills

• Prior home health occupational therapy experience
• Familiarity with home health documentation and compliance requirements
• Strong communication and patient education skills
