RN Outpatient Case Manager

Job Summary:

The RN Outpatient Case Manager is responsible for coordinating inpatient and outpatient admission processes and facilitating access to healthcare services. This role involves managing case workflows, collaborating with healthcare teams, and ensuring effective patient care coordination. The position requires strong case management experience and the ability to work within a multidisciplinary environment. Responsibilities:

• Coordinate inpatient and outpatient admissions and access to healthcare services.
• Manage case workflows and patient care plans.
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes.
• Maintain accurate documentation and communication regarding patient status and care coordination.
• Utilize electronic health record systems, including Epic, for case management activities.

Required Skills & Certifications:

• Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program.
• Active Registered Nurse (RN) license in the state of practice.
• Minimum of 3 years of RN Case Management experience.
• Experience with hospital case management and Epic electronic medical record system.

Preferred Skills & Certifications:

• Strong communication and interpersonal skills for effective collaboration.
• Experience working in outpatient and inpatient healthcare settings.

• Position is based in an outpatient case management setting.
• Potential housing available nearby for travelers during assignment, subject to availability.

Scheduling:

• Full-time position with a 40-hour workweek.
