Are you a driven RN or LPN looking to care for pediatric patients in their homes?

 

H2 Homecare is searching for nurses who are dedicated to providing care for patients located throughout Georgia. We support our nurses through on-site training with our clinical supervisors, as well as a supportive team that will work with you to provide flexible hours, a supportive orientation, and training on our EMR (electronic medical record) system.

Credentials:

• Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the State of Georgia
• Graduate of an accredited practical nurse or vocational nursing program
• One year of prior professional nursing experience, homecare experience preferred
• Current CPR certification
• Clean background check

Job responsibilities:

• Changes in the patient’s condition are identified and documented with resolution on the appropriate clinical form
• Physician notification to obtain necessary orders for intervention(s) per state regulations
• Performs specific treatments and medication administration in accordance with physician orders
• Documentation reflects patient/family collaboration and participation in their plan of care
• Documentation and clocking in and out utilizing the current patient health record platform, Alayacare
• Skilled observations and significant changes in patient status/care are communicated to the Director of Clinical Services and/or Clinical Supervisor/Case Manager
• Request for modification of orders and additional orders are made based on the patient’s clinical status and Skilled nursing judgment
• All physician collaboration /coordination of care is documented in the medical record and communicated as needed
• Adheres to H-squared Homecare policies and procedures
