Practice Info Senior-focused primary care practice Dedicated to delivering high-quality, relationship-based care for adults age 65 and older Value-based, managed care environment Smaller patient panels (~500 patients) Extended visit times (30–45 minutes) Dedicated clinical and administrative support staff Responsibilities Provide comprehensive primary care services to Medicare Advantage and senior patients age 65 Perform thorough assessments and develop individualized care plans Manage chronic conditions, preventive care, and acute medical concerns Deliver patient-centered care within a managed care/value-based care model Collaborate with physicians, specialists, and interdisciplinary care teams to coordinate treatment plans Identify and close care gaps while supporting quality and outcome initiatives Educate patients and caregivers regarding disease prevention, healthy lifestyle choices, and long-term wellness Build strong relationships with patients and families through compassionate, personalized care Compensation Open Compensation Salary: $260K Partnership Opportunity after 2 Years with potential for significant additional earnings Benefits Health, Dental & Vision Insurance Generous PTO and CME Allowance 401(k) Retirement Plan Fully Funded Malpractice Coverage Relocation Assistance Available Shift & Schedule Monday – Friday 8:00 AM – 5:00 PM Requirements MD or DO degree from an accredited medical school Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics Experience in Managed Care, Value-Based Care, or Medicare Advantage preferred New Graduates Welcome Florida Medical License preferred or eligible ACN licensed accepted Geriatric care experience preferred Strong communication and patient engagement skills Spanish-speaking ability preferred for one openingaa415a4b-8b21-40fc-a65c-70d2b25ca29a
