Provide sensitive, resident-centric daily personal care to patients in a long-term care setting, ensuring comfort, dignity, and peace. Patient Hygiene & Comfort:

Assist with personal hygiene, grooming, bathing, light nutrition, and repositioning/safe transfers. Measure and record vital signs. Family Support & Communication:

Serve as a comforting, professional presence, reporting any clinical changes or urgent needs promptly to the hospice nursing supervisor. Environment Maintenance:

Assist with maintaining a clean, calm, and supportive home environment for the patient and their visiting loved ones. Technology Proficiency:

Demonstrated ability to navigate MatrixCare efficiently with your cellphone, ensuring timely and accurate documentation of patient care, timesheets, and travel time. Ability to Travel (when needed):

Capable of independent travel to various local client residences, when needed, and maintaining detailed and precise records of travel time and destinations. Requirements Must possess an active

Pennsylvania CNA certification Minimum

6 months

of hospice, home health, skilled-nursing, or related clinical experience Highly punctual, reliable, and trustworthy A deeply caring, empathetic, and resilient attitude suited for home hospice environments A reliable vehicle and Driver's License
