Social Services Director Northcrest Nursing & Rehabilitation Center | Napoleon, OH Full-Time Join a Great Place to Work® Certified Team! Northcrest Nursing & Rehabilitation Center is seeking a compassionate, organized, and resident-focused Social Services Director to join our leadership team. This position plays an important role in supporting our residents and families while helping ensure a positive experience from admission through discharge. Key Responsibilities: Lead and oversee the facility’s Social Services department Advocate for residents and support their psychosocial and emotional needs Build strong relationships with residents, families, and responsible parties Participate in care planning and interdisciplinary team meetings Coordinate discharge planning and safe transitions of care Connect residents and families with community resources and services Address resident and family concerns promptly and professionally Complete required assessments, documentation, and care plan updates Support resident rights and regulatory compliance Collaborate with Nursing, Admissions, Therapy, Activities, and facility leadership Qualifications: Degree in Social Work, Human Services, or a related field preferred Previous social services experience in skilled nursing, long-term care, healthcare, or a related setting preferred Knowledge of resident rights and discharge planning is a plus Strong communication, organization, and problem-solving skills Compassionate, professional, and team-oriented Why Northcrest?
