Providence Center Child & Family Clinical Therapist - Fee For Service

The Providence Center Child & Family Clinical Therapist - Fee For Service will provide outpatient psychotherapy either at The Providence Center or in the community. Perform a comprehensive psychosocial assessment of assigned consumers.

Complete crisis assessments, initial assessments, and integrated summary forms.

Develop and implement a plan of care with ongoing consumer input.

Provide individual mental health and/or substance abuse treatment and crisis assessment.

Perform record keeping in accordance with Health Information Services and other Canter requirements.

Master's degree in social work or related field and independent license required (LICSW and/or LMHC).

Must have insured auto and valid driver's license.
