If you want to make a difference, we want to hear from you.Job SummaryKey ResponsibilitiesProvide oversight and direction for weekend case management operations, ensuring effective workflow and timely service deliveryServe as a liaison between department leadership, case managers, and support staff to promote collaboration and problem resolutionSupport leadership in the development, implementation, and refinement of processes aligned with departmental and organizational goalsMonitor and evaluate workflows and outcomes to identify opportunities for continuous improvementAssist in implementing operational and process changes to enhance efficiency and quality of carePartner with leadership to develop and maintain training and educational resources that support staff competency and professional growthEnsure adherence to regulatory, accreditation, and legal requirements related to case management practicesProvide guidance and support to weekend staff, addressing escalations and operational challenges in real timePerforms interval departmental audits, and oversight of on-going processes that support the department’s established performance improvement planFacilitate mandatory weekend huddles to proactively identify and resolve system-wide barriersProvide direction, assistance, and caseload coverage for the department during staff shortagesQualificationsBSN or MSW requiredCurrent Georgia Registered Nursing( RN) license required if BSN.CCM, LMSW, or LCSW preferredMinimum of 3 years of experience in case management, social work, or care coordination requiredStrong knowledge of regulatory and compliance standards in healthcareExcellent communication, collaboration, and problem-solving skillsAbility to lead in a fast-paced, dynamic environment with minimal supervisionAbility to effectively manage multiple tasks, activities, and responsibilitiesMust work every Fri/Sat/Sun or Sat/Sun/MondayCore CompetenciesThese competencies reflect the values and behaviors expected of all Grady team members, regardless of role.
