Registered Nurse LTC/SNF

Call for Salary Rate -

Job Summary:

Provides comprehensive nursing care to residents in a long-term care or skilled nursing facility setting. Collaborates with healthcare team members to ensure high-quality patient outcomes and safety.

Responsibilities:

• 
  Assess residents' health status and develop individualized care plans.
  
  
• 
  Administer medications and treatments according to physicians orders.
  
  
• 
  Monitor patient conditions and document changes accurately.
  
  
• 
  Educate residents and families on health management and care procedures.
  
  
• 
  Ensure compliance with all facility policies, safety standards, and regulatory requirements.
  
  
• 
  Collaborate with interdisciplinary team to coordinate patient care and improve outcomes.
  
  

Qualifications:

• 
  Valid RN license in the state of practice.
  
  
• 
  Proven experience in LTC or SNF environments preferred.
  
  
• 
  Strong assessment and clinical skills.
  
  
• 
  Ability to work effectively in a team setting.
  
  
• 
  Excellent communication and documentation skills.
  
  
• 
  Current CPR and BLS certifications required.
