You’ll serve as a case manager and ensure quality, compassionate care in the home setting.Key Responsibilities:Complete initial and ongoing patient and family assessmentsDevelop and revise individualized hospice care plansProvide professional nursing care using the nursing processAdminister medications and treatments as prescribedEducate and counsel patients and families on hospice careCoordinate care for your assigned caseloadWhat We’re Looking ForActive CA RN LicenseMinimum 2 years of nursing experience (hospice or home health experience preferred)Strong clinical, communication, and problem-solving skillsCurrent CPR certificationValid driver’s license, insured vehicle in good conditionAbility to work independently and manage prioritiesWhy Join Sequoia?Sequoia Hospice is part of the Cornerstone Group, a network of 75 agencies nationwide.
