Specific Responsibilities: Assessment & Evaluation: Conducting thorough physical exams of wounds to document their size, depth, drainage, tissue health, and underlying causes. Treatment Implementation: Executing individualized care plans. This includes debriding dead tissue, applying specialized dressings, and setting up therapies like Negative Pressure Wound Therapy (NPWT). Patient & Caregiver Education: Teaching patients how to change dressings, recognize early signs of infection, and implement lifestyle or positioning changes to prevent further skin breakdown. Multidisciplinary Collaboration: Partnering with physicians, physical therapists, dietitians, and social workers to address underlying systemic issues (e.g., nutrition, circulation, mobility) that impede healing. Prevention Strategies: Identifying at-risk patients (like bedbound individuals) and implementing preventive protocols (e.g., turning schedules, special support surfaces) to avoid pressure injuries. Documentation & Compliance: Accurately charting wound progression and ensuring treatment protocols meet facility and healthcare regulations.
