INTAKE COORDINATOR

Required Schedule: Monday -Friday with alternating weekends

Job Summary: To ensure the daily communication to the team and complete clinical administrative duties for the organization of the interdisciplinary team.

Job Duties:

• Maintains positive and effective communication with all employees, referral sources, and customers.
• Follows the Avow Compliance and Ethics Plan.
• Ensure confidentiality of all clients, family, and employee information in accordance with HIPAA and company policies.
• Process new referral information into the electronic medical record (EMR) system in a timely manner.
• Manage and process all incoming referrals and intake data received via platforms such as Focura, Epic, Pointcare, and other communication channels.
• Enter all referral information into the WellSky ERM accurately, ensuring proper documentation and coding.
• Sort, Separate, and label incoming referral documentation, including H&P (History & Physical), and Face-to-Face (F2F) evaluations, and organize them into appropriate patient files.
• Conduct welcome calls to all new referrals to verify demographic data, gather access instructions (e.g., door/gate codes), and update information in WellSky ERM accordingly.
• Maintain and monitor adequate inventory of Start of Care (SOC) packets; distribute to assessing clinicians as required.
• Gain and maintain proficiency in Forcura, WellSky, and Adobe Sign to manage digital documentation and referral workflows efficiently.
• Prepare, send, and distribute outgoing and incoming faxes as needed.
• Monitor inventory and reorder SOC folders to ensure supply meets demand.
• Set up and ordering of wound vac for clients with orders.
• PT/INR oversight to include faxing results to MDs daily.
• Telehealth Oversight / trend monitoring.
• Provide support to the Scheduler and other ream members as needed to ensure smooth operations.
• Adhere to all company policies, including compliance and ethics program guidelines.
• Perform all other duties as assigned to support the Intake and Clinical Operations team.

Education/Experience: Associate degree preferred; or one to two years related experience and/or training; or equivalent combination of education and experience preferred. Medical office experience preferred.

Computer Skills: Knowledge of basic typing skills, data entry and word processing software. Database software and general knowledge of e-mail is preferred.

Language Skills: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
