Traveling Vascular RN Case Manager - Casual Role

Fairview Health Services in Minnesota is seeking an RN Case Manager for the Vascular Clinic. This casual role travels to vascular clinics in Maplewood, Woodbury, and Wyoming, and to surgery centers in Minneapolis and Maple Grove, with work hours from 7:30 a.m. to 5:00 p.m.

You will develop and implement care plans, triage nursing calls, perform wound and vascular assessments, and educate patients while collaborating with physicians, LPNs, CMAs, and caregivers.
