Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of home care and interact with other medical disciplines as needed. You will conduct admission visits to assess eligibility and develop personalized care plans for patients in the Home Health setting. Requirements include an RN license, current CPR, a valid driver’s license, and reliable transportation with auto liability insurance; one year of RN experience is preferred. #J-18808-Ljbffr Angels Care
