This position is primarily remote and home-based; however, candidates must reside in the Nashville, Tennessee area. We're looking for a highly organized, patient-focused professional to join our team as a Patient Coordinator. About the Role Patient Coordinators are often the first voice patients hear when they begin their journey with Custom Health. This is a highly interactive role where you'll spend much of your day speaking with patients on the phone, helping them understand our services, answering questions, and guiding them through the enrollment process. You'll build relationships, create positive first impressions, and help patients feel confident about taking the next step. Behind every successful patient conversation is a lot of coordination happening in the background. You'll manage patient expectations, track activity through multiple systems, coordinate with pharmacy partners and internal teams, schedule follow-ups, and help ensure patients continue moving smoothly through the process. What You'll Do Build Patient Relationships • Serve as the first point of contact for prospective patients. • Contact referred patients and consumers who have requested more information. • Explain Custom Health services in a clear, supportive, and professional way. • Build trust and rapport with patients and caregivers. • Answer questions and provide education regarding available programs. • Deliver a positive and supportive patient experience. Qualify & Enroll Patients • Assess patient eligibility, interest, and program suitability. • Verify coverage and help patients understand available services and next steps. • Support enrollment and onboarding readiness. • Identify and address barriers that may prevent enrollment. • Help patients feel confident and prepared as they move toward onboarding. Coordinate Patient Progression • Manage patient progression through qualification. • Maintain accurate records within CRM systems and patient tracking tools. • Coordinate next steps with pharmacy partners and internal teams. • Organize and track patient progress through the onboarding process. • Manage follow-up schedules to maximize outreach and enrollment opportunities. • Ensure patients are prepared for a successful onboarding experience. Support Growth & Process Improvement • Maintain high levels of patient engagement and responsiveness. • Support strong relationships with patients, caregivers, and partner organizations. • Help refine and improve processes that create a more seamless onboarding experience. • Monitor outreach and enrollment performance. • Contribute to reporting, tracking, and operational best practices. • Support ad hoc projects and duties as needed. Requirements • 3+ years of experience in healthcare, patient coordination, customer care, social work, or a related field. • Exceptional communication and relationship-building skills. • Professional telephone presence and comfort managing patient-facing conversations. • Strong organizational and time management abilities. • Comfortable managing a high volume of patient interactions and follow-ups. • Experience working with CRM systems preferred. • Strong attention to detail and follow-through. • Proficient with computers, software programs, and remote work tools. • Comfortable working independently in a remote environment.
