Requirements
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an accredited program - Required

Roles & Responsibilities
- Perform clinical assessments to identify patient needs and determine appropriateness for therapy.
- Document, prepare, and review patient care plans, progress notes, and other clinical records on an ongoing basis.
- Plan and deliver suitable treatments for neuromuscular, neurological, and orthopedic disorders based on patient evaluations.
- Educate patients and their families regarding therapeutic procedures appropriate to their condition and readiness to learn.
- Collaborate with an interdisciplinary team to provide comprehensive patient care.
- Apply concepts of infection control and universal precautions.

Compliance / Notes
- Must meet mandatory continuing education requirements.
- Must be able to work flexible hours and travel locally.
