Work Environment\n

This position offers a flexible work environment, primarily conducted remotely, allowing you to engage with clients and coordinate care from the comfort of your home. However, you will also be expected to conduct in-home visits and meetings as necessary. Our operating hours are Monday through Friday from 8:00 AM to 4:30 PM, with occasional needs for evening or weekend availability based on client requirements. Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs by exploring emotional, mental, and social factors influencing their well-being.

• Develop collaborative, personalized care plans in partnership with seniors and their families, focusing on achieving specific goals related to their quality of life.

• Provide counseling and emotional support to clients, guiding them through transitions, resolving conflicts, and managing challenges they face.

• Advocate for clients’ rights and facilitate access to community resources by connecting them with appropriate support services.

• Collaborate with an interdisciplinary team, including nurses, caregivers, and healthcare professionals, to enhance the holistic care offered to clients.

• Perform regular client visits to monitor well-being, assess living situations, and address any safety concerns.

• Educate clients and families about available resources, fostering social engagement and a community-oriented approach.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 60,000 to 100,000 per year, commensurate with experience and qualifications.
