Job Description
Title: Physical Therapist - Rehabilitation

Requirements:
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the state of practice.
- Experience in skilled nursing facility (SNF) or geriatric rehabilitation preferred.
- Strong clinical, communication, and organizational skills.
- Ability to work both independently and as part of an interdisciplinary team.

Roles & Responsibilities:
- Perform initial and ongoing assessments of residents' mobility, balance, strength, gait, and functional limitations.
- Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability.
- Provide direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and use of physical agents or modalities.
- Monitor residents' progress and adjust care plans as necessary to ensure achievement of therapy goals.
- Participate in interdisciplinary care team meetings and contribute to discharge planning as appropriate.
- Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements.
- Accurately document treatment sessions, progress notes, and billing information in a timely manner.
- Ensure compliance with all state, federal, and facility documentation and practice standards.
