Declaration of Applicant The following affidavit is offered to satisfy the requirement of Texas Education Code section 21.009 for a pre-employment affidavit, in accordance with Texas Civil Practices and Remedies Code section 132.001. I hereby authorize the Grand Prairie Independent School District to use this document to obtain from my current or past employer any personnel information, employment records, or other information, written or oral, relating to my qualification for employment and job performance. Submitting this form indicates my request and authorization to any current or former employer of mine to disclose information about my job performance to Grand Prairie Independent School District pursuant to Chapter 102 of the Texas Labor Code.
