Your responsibilities include:Evaluating patient functional levels and collaborating with physicians to develop individualized plans of carePreparing clinical notes, progress notes, and documentation in accordance with regulatory standardsProviding therapeutic treatments and interventions based on the plan of careConsulting with families and agency staff to support patient goalsParticipating in in‑service programs and interdisciplinary collaborationAssessing equipment needs and ensuring patients receive appropriate assistive devicesReviewing and revising plans of care as needed (at least every 60 days)Practicing infection control and universal precautionsReporting suspected abuse according to state and federal guidelinesPlanning for patient discharge and evaluating treatment effectivenessYou take pride in delivering high‑quality, compassionate care that truly impacts patients' lives.QualificationsGraduation from an accredited occupational therapy programOR eligibility for the national registration examOR at least 2 years of OT experience with a satisfactory proficiency examCPR certification within 60 days of hireAbility to lift up to 50 poundsWe're looking for someone who is friendly, respectful, dependable, and committed to professionalism and teamwork.Ready to Join Our Home Health Care Team?Our application process is quick and simple.
