The Medical Social Worker Hospice plays a vital role in providing compassionate support and comprehensive care planning for patients in hospice settings. This position involves direct patient interaction to assess needs, provide counseling, and collaborate with families and healthcare teams to ensure holistic care during end-of-life stages. The role requires local travel within the region and proficient use of Electronic Health Records (EHR) for documentation.

Responsibilities

• Conduct patient assessments to identify psychosocial needs
• Develop and implement individualized care plans
• Provide counseling support to patients and families
• Serve as a liaison between patients, families, and healthcare teams
• Intervene during crises to offer immediate support
• Document all assessments, interventions, and communications in EHR
• Collaborate with interdisciplinary teams to coordinate care

Required Qualifications

• Bachelor's degree in Social Work (BSW)

Preferred Qualifications

• 2+ years of experience in medical social work
• Strong patient advocacy skills
• Expertise in crisis intervention and care coordination
• Excellent communication and empathy
• Sound ethical decision-making abilities
• Proficiency in documentation and use of Electronic Health Records
• Cultural competence in diverse patient populations
