Registered Nurse Pre Admission Testing

Position Summary We have an exciting opportunity to join our team as a Counselor - Fulltime - Monday - Friday - 9am - 5 pm - Brooklyn. Job Responsibilities Engage dental patients to assess for outpatient health needs Assist dental patients in accessing outpatient health services Screen for social determinants of health Address barriers to accessing health services Screen patients for eligibility for public benefits and other programs and resources Assist clients with benefit applications Make referrals to a variety of internal and external programs that provide food, shelter, clothing, legal services, educational and vocational services, and other needed services Complete documentation in EPIC as required Attend staff meetings and other required meetings Counsel patients on available resources and provide advocacy Provide education and advocacy to help patients navigate services Strengthen relationships with community partners to identify resources for clients Minimum Qualifications To qualify you must have a Bachelor's Degree preferred 1+ years relevant experience working, ability to plan and manage Knowledge of public benefits History of immigrant community engagement Preferred Qualifications Excellent communication skills, ability to multi-task, experience in providing services to a diverse community, ability to be a team player Qualified candidates must be able to effectively communicate with all levels of the organization.
