Compassionate Hospice RN Case Manager

Gardens of Western Reserve Hospice is seeking an RN Case Manager / LPN to coordinate patient care across home-based settings in the Cuyahoga Falls area. You will manage a caseload, provide bedside care, symptom management, and collaborate with an interdisciplinary team. The position requires Ohio RN or LPN licensure with at least 2 years of nursing experience; hospice experience is a plus. Travel to patients in Summit/Portage counties is required, along with on-call coverage and benefits #J-18808-Ljbffr Agupdate
