Speech Therapist

Speech Language Pathologist

Under general supervision is responsible for providing speech, language, and dysphagia services to patients include; evaluations, planning individual patient care programs, treatments, and home programs.

Essential Duties and Responsibilities

Performs patient evaluations and administers treatment for speech language and swallowing disorders.

Designs and revises plans of care and goals.

Provides written and/or verbal reports to physicians.

Completes documentation according to unit standards and regulatory requirements.

Participates in in-service education programs.

Provides instruction, direction and supervision to new employees.

Keeps abreast of current trends and developments in speech and swallowing disorders

Participates in related activities such as patient care committees, case conferences, personnel orientations, community education projects and in-service education.

Attends staff meetings.

Provides families of patients with information concerning specific disorders as needed

Review each patients chart and history to become familiar with patients' needs

Develops patient literature with other members of the department

Maintains contact with all referring physicians of patients in the programs and sends discharge summary to the physician after program is completed

Assists patients to obtain home therapy equipment

Attends and contributes to in-services and comprehensive plans of care for the patients of the Agency

Assists with quarterly clinical records review

Uses equipment and supplies efficiently and effectively

Provide reliable transportation to and from work and patients' homes

Pursues personal professional growth and development through participation in education programs and review of current health care literature

Understand and adheres to all policies, philosophies and standards of the Home Health Care Agency

Any other duties as assigned

Job Requirements

Education: Master's degree in speech pathology.

Licensure/Certification: Current licensure by the Indiana Board of Examiners on Speech Pathology and Audiology required.

Experience: Two - three years' experience in clinical speech pathology. Excellent interpersonal skills required. May be assigned to work in homes of patients from all socio-economic levels.
