Behavioral Health Manager (LCSW) (Los Angeles, CA)

The Behavioral Health Manager will lead the Social Work department at myPlace Health's downtown Los Angeles clinic, guiding a team within the PACE program that supports high‑risk seniors in their homes and communities. Responsibilities Lead with purpose and direction, overseeing the Social Work department by setting clear goals, priorities, and performance standards aligned with myPlace Health’s mission and PACE program requirements. Inspire and empower the team, supervising and mentoring Social Workers and Mental Health Counselors, fostering accountability, teamwork, and continuous professional growth. Build strong operational rhythms, managing team schedules, coverage, onboarding processes, performance management, and skill development. Use data to drive impact, leveraging analytics and key performance indicators to identify trends, inform decisions, and lead improvements in service delivery. Champion clinical excellence, ensuring timely, high‑quality completion of biopsychosocial assessments, individualized care plans, and social work interventions that reflect compassion and regulatory compliance. Streamline processes for consistency, establishing and refining workflows, policies, and procedures that ensure consistent, participant‑centered care across multiple sites. Guide through complexity, supporting high‑acuity cases, crisis intervention, and transitions of care to safeguard participant well‑being and stability. Serve as a trusted expert, providing guidance on trauma‑informed care, advanced care planning, grief support, and addressing social determinants of health. Lead by example in care delivery, maintaining a small participant caseload to stay closely connected to day‑to‑day social work practice. Uphold integrity through compliance, ensuring all practices meet CMS, DHCS, and PACE standards and maintaining ethical and regulatory accountability. Elevate quality and performance, overseeing EMR documentation for completeness and accuracy while leading audits, surveys, and corrective action planning to drive continuous quality improvement. Collaborate beyond boundaries, partnering with Nursing, Therapy, Enrollment, and Center Operations to ensure seamless, interdisciplinary care for every participant. Advocate for alignment and integration, representing the Social Work function in organizational planning, operational readiness, and strategic growth initiatives. Steward resources wisely, managing resource allocation and vendor partnerships related to social work services to support efficient and effective care delivery. Adapt and contribute, performing other responsibilities as needed to support organizational goals and participant‑centered outcomes. (Optional) Pursue opportunities to utilize AI and other innovations to enhance operations and member care, and support SCAN’s Vision and Goals. Qualifications Master’s degree in Social Work (MSW) from a CSWE‑accredited program and an active California license as an LCSW. At least 5 years of progressive social work experience, including 3 or more years in a leadership or management capacity, ideally within healthcare, PACE, or senior‑care environments. Deep expertise in supporting older adults, high‑risk seniors, and individuals with complex medical, behavioral, and psychosocial needs. Solutions‑oriented mindset, balancing long‑term vision with day‑to‑day operational realities. Outstanding verbal and written communication skills. People‑first approach, building trust through active listening, thoughtful feedback, and fairness. Ability to collaborate effectively in cross‑function teams (Nursing, Therapy, Operations, Enrollment). Commitment to advancing health equity and ensuring individualized, dignified care. Motivation to mentor others, pilot new approaches, and model curiosity and adaptability. Strong commitment to service, respect, and excellence, bringing meaning to leadership each day. Valid California driver’s license and willingness to travel between sites as needed. Schedule This role is 100% onsite at our downtown Los Angeles clinic. Standard schedule: Monday through Friday, allowing for dependable structure while fostering meaningful daily connections. Tuberculosis Screening Policy To ensure the health and safety of our members, prospective employees must provide proof of Tuberculosis screening upon hire or proof of a negative screening within the last year. Benefits Base salary range: $106,200 to $153,705 per year. Annual employee bonus program. Medical, Dental, Vision coverage.
