Case Manager

Case Manager Market leading managed care organization looking for Case Managers to work in a hybrid role! Must have a license or certification for this role. Location & Travel Working from home and traveling in the field in the Cook, Lake, Will, Kane, Winnebago, Rock Island, IL areas (mileage reimbursed). Shift Monday–Friday, 8 hour day shift. Contract Contract with potential for long term based on performance. Responsibilities Through the use of clinical tools and information/data review, conduct comprehensive assessments of referred member's needs/eligibility and determine approach to case resolution and/or meet needs by evaluating member's benefit plan and available internal and external programs/services. Apply and/or interpret applicable criteria and guidelines, standardized case management plans, policies, procedures, and regulatory standards while assessing benefits and/or member's needs to ensure appropriate administration of benefits. Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures. Qualifications License or certification required for the role.
