About the Role:

Join our dedicated home health team in Upper Darby Township, PA as a Home Health Program Specialist, where you'll play a vital role in delivering compassionate, high-quality care to patients in the comfort of their own homes. Responsibilities:

• Coordinate and oversee home health programs to ensure seamless delivery of patient care services
• Collaborate with clinical staff, physicians, and care teams to develop and implement individualized patient care plans
• Monitor program compliance with state, federal, and accreditation regulations for home health services
• Maintain accurate patient records, documentation, and reporting in accordance with home health standards
• Serve as a liaison between patients, families, and healthcare providers to facilitate communication and continuity of care
• Identify opportunities for program improvement and support quality assurance initiatives
• Assist with intake, scheduling, and coordination of home health visits and services
• Attending ISP & Monitoring meetings

Requirements:

• Experience in home health, healthcare coordination, especially with Adults with Intellectual disability
• Knowledge of home health regulations, Medicare/Medicaid guidelines, and compliance standards
• Strong organizational and multitasking skills in a fast-paced healthcare environment
• Excellent communication and interpersonal skills for working with patients, families, and clinical teams
• Proficiency with electronic health records (EHR) and healthcare management software
• Bachelor's degree in healthcare administration, Sical work, nursing, or a related field preferred
• Compassionate, detail-oriented, and patient-focused approach to care coordination
• Clean Driver's License
