Spaulding Nursing and Therapy Center Brighton, Inc. is seeking a full‑time Case Manager (M‑F day shift) to join our interdisciplinary team. You will conduct intake interviews, determine need for evaluations, review care plans, and coordinate services to support post‑inpatient and home health transitions. The ideal candidate has case management or discharge planning experience, SNF exposure, and a MA license (RN, PT, OT, SLP, RT or LCSW/LICSW).
