Physical Therapist

Join Our In-House Therapy Team at The Center for Living and Rehab! We are currently seeking a PRN Physical Therapist (PT) to join our in-house rehab team. At The Center for Living and Rehab, we take pride in delivering high-quality, resident-centered care in a supportive and team-oriented environment. Responsibilities Evaluate residents and develop individualized physical therapy treatment plans Deliver skilled therapy services to help improve mobility, strength, and overall function Monitor progress and adjust treatment plans based on resident response Educate residents, caregivers, and staff on therapy goals and safety techniques Document evaluations, treatment sessions, and outcomes accurately and timely using electronic medical records Collaborate with interdisciplinary team members to support resident care plans Qualifications Graduate of an accredited Physical Therapy program Active Vermont Physical Therapist license (required) Previous experience in SNF or long-term care preferred Strong communication, organizational, and clinical skills Experience with EMR systems preferred The Center for Living and Rehab is an Equal Opportunity Employer – AA/EEO/M/F/D/V #J-18808-Ljbffr Centerforlivinghc
