About the Position

TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in San Diego, California, 92122!

Job Title: Field Registered Nurse Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:30 - 17:00) Experience: Minimum 1 year Home Health experience required License: California RN License in hand Certifications: BLS required Must-Have: OASIS experience required HomeCare HomeBase experience required Previous Home Health or Hospice experience required Description: Position available for a Field Registered Nurse. This role covers a North San Diego territory. The schedule is Monday to Friday, with one weekend per month required. The weekly schedule will be adjusted accordingly for the weekend worked. The average number of patients seen per day is 5-7. Cases primarily involve home health care for geriatric patients. First-time home health nurses are welcome to apply. On-call requirements do not apply to this position. Travel will be within a 50-mile radius from the hub office. Mileage will be reimbursed at the IRS standard rate between patient homes. Locals are accepted at the travel rate. Experience with electronic medical record systems is necessary. Skills required include admission assessments, clinical assessment documentation, and discharge planning. Additional competencies include infection control precautions and IV maintenance. Ideal candidates should demonstrate effective case management and coordination of services. Home health experience is a critical qualification for this position.
