Position Purpose The Palliative Care RN, Registered Nurse, is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation. Position Purpose The Palliative Care RN, Registered Nurse, is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation. What You Will Do Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy Provides statistical/budgetary data to appropriate Palliative care program Works collaboratively with all departments to ensure execution of quality delivery of services Ensures that stakeholders are satisfied with services provided; improvement plan developed when necessary Develops tracking outcomes of enrolled patients and ensures information is available for review A commitment to quality palliative care provision and transition to hospice when appropriate Exercises sound clinical judgment in oversight of Palliative Care Program Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions Minimum Qualifications Graduate of an approved nursing education program. Current Registered Nurse licensure in the state of practice. CPR certification 2-3 years acute care nursing and Hospice/Palliative Care certification preferred.
