The Pennant Group, Inc. is seeking an Advantage Case Manager in Edmond, OK to oversee case management for aging or disabled populations. You will work under CMS management, manage intake, assessment, service plans, and discharge while coordinating resources and ensuring regulatory compliance. Qualifications include RN or LPN with CACM or a Bachelor's degree with CACM and one year of relevant experience.
