Certified Nurse Practitioner (CNP) – Virginia

We are seeking a compassionate and experienced Certified Nurse Practitioner (CNP) to provide high-quality, patient-centered care in a collaborative healthcare environment. This role offers the opportunity to practice with a high level of clinical autonomy while delivering comprehensive primary and preventive healthcare services.

The Nurse Practitioner will perform patient assessments, diagnose and treat acute and chronic conditions, prescribe medications (including controlled substances in accordance with state regulations), develop treatment plans, and provide health education to patients. The ideal candidate is a confident clinical decision-maker who is committed to delivering exceptional care and improving community health outcomes.

Key Responsibilities

• Perform comprehensive patient evaluations, including obtaining medical histories and conducting physical examinations.
• Diagnose and treat acute and chronic illnesses.
• Order, perform, and interpret diagnostic and laboratory tests.
• Develop individualized patient care and treatment plans.
• Prescribe medications, including controlled substances, in accordance with Virginia law.
• Perform or oversee nursing assessments and clinical procedures.
• Provide patient education, counseling, and preventive health services.
• Coordinate patient care through case management and appropriate referrals.
• Collaborate with healthcare professionals to ensure continuity of care.
• Maintain accurate and timely electronic medical records.
• Participate in quality improvement initiatives, continuing education, and professional development.

Required Qualifications

• Active Virginia Registered Nurse (RN) license.
• Current Virginia Certified Nurse Practitioner (CNP) license.
• Current CPR/BLS Certification.
• Strong knowledge of:
  • Advanced nursing practice
  • Physical assessment techniques
  • Disease processes and pharmacology
  • Diagnostic testing and interpretation
  • Differential diagnosis and treatment planning
• Ability to independently assess, diagnose, and manage patient care.
• Excellent clinical judgment with the ability to recognize limitations and seek consultation when appropriate.
• Strong communication, interpersonal, and patient education skills.
• Ability to work independently with minimal direct supervision while collaborating effectively with interdisciplinary teams.
