Job Summary We are seeking a highly motivated and organized PAS Care Manager to join our team. In this role, you will play a crucial role in coordinating patient care, collaborating with healthcare professionals, and ensuring optimal patient outcomes. This position requires strong communication, organizational, and problem-solving skills, as well as a passion for patient-centered care. Responsibilities Patient Care Coordination: Collaborate with physicians, nurses, and other healthcare professionals to develop and implement comprehensive patient care plans. Communication and Collaboration: Maintain clear and effective communication with patients, families, and healthcare team members; facilitate communication and collaboration among different departments and disciplines; participate in team meetings and rounds to ensure coordinated care. Documentation and Record Keeping: Maintain accurate and up-to-date patient records and documentation; ensure compliance with relevant regulations and standards. Resource Management: Identify and access appropriate resources for patients and their families; advocate for patient needs and ensure access to necessary services. Other Duties as Assigned: Perform other duties as assigned by the supervisor. Qualifications At least two years of experience in care management, healthcare, or home care settings is preferred. Licensed Vocational Nurse Social Worker Skills Strong communication, interpersonal, and organizational skills. Ability to work independently and as part of a team. Proficiency in electronic health record (EHR) systems. Knowledge of relevant healthcare regulations and standards.
