Social Worker

Countryside Health Care of Milford is seeking a Memory Care Unit Social Worker to join our team on a part-time basis (24 hours per week). The role involves planning, developing, and directing social services within the memory care unit, and coordinating policies and procedures. You will interview residents and families, assist in discharge planning, and provide guidance on Medicare/Medicaid and financial assistance options.
